Call our dedicated doctors’ line at (502) 403-1111 for quick and easy scheduling. Please note that for emergency appointments, referring doctors must call this number to schedule.
Option 1
Print and fax the Patient Referral Form, along with the exam notes, to (502) 897-9824. A representative from our office will contact you to schedule the appointment with the patient. For emergency appointments, please call the office directly.
